Healthfirst Project Coordinator, HQIP in New York, New York

Description

The Project Coordinator will play an integral role in collaborating with Healthfirst contracted Licensed Home Care Service Agencies, as well as supporting the Clinical Performance Manager who is responsible for oversight of the Healthfirst Quality Incentive Program for Licensed Home Care Service Agencies.

  • Schedule in-person and virtual meetings with home care agencies to review their current quality metric ratings

  • Document key action items the home care agencies will implement in order to improve performance, and regularly follow-up with agency staff to ensure actions are completed

  • Assist home care agencies is getting connected to the Healthfirst Home Health Incentive Dashboard, a tool agency staff use to track their quality measure ratings, member information, and incentive bonus earnings

  • Prepare and distribute monthly reports to support home care agencies in improving their quality metric ratings

  • Collaborate interdepartmentally on initiatives that involve home care agency outreach

  • Conduct Patient Care Aide/Home Health Aide best practice research, and disseminate information/tools to home care agencies

  • Provide additional administrative support as necessary including regularly updating home care agency contact information

Minimum Qualifications

  • Associate‚Äôs degree

Preferred Qualifications

  • Previous experience or knowledge of the Licensed Home Care Service Agency industry

  • Previous experience or knowledge of Managed Long Term Care and /or Value Based Payment programs

  • Intermediate Outlook, Excel, and PowerPoint skills

  • Ability to work independently in a fast paced environment
